Job Description:
Job Summary: The Patient Navigator - Grants promotes Patient and Family Centered Care principles to patients, families, staff, providers and visitors as they navigate through the JPS Health Network.
Essential Job Functions & Accountabilities:
- Listens to patients' and families' feedback about level of care and/or facility conditions; determine appropriate health care team member or department, communicate concerns and/or issues and facilitate resolution. Provide timely feedback to patient.
- Communicates with patients, families and other visitors to inform them about policies, protocols, rights and responsibilities; coach staff and providers regarding communication styles and cultural perspectives for best patient interaction potential.
- Promotes and make recommendations for process changes to improve overall patient and family experience in JPS system.
- Implements educational programs to support Patient & Family Centered Care principles.
- Maintains professional relationships with community organizations to promote and inform the public about JPS Health Network and its services.
- Recognizes the quality improvement and participation/initiation of the improvement process that meet the needs and expectations of patients/families.
- Provides benefits/entitlement counseling and referral to health care services to assist patients with accessing other public and private programs for which they may be eligible.
- Educates patients about available benefit programs, assesses eligibility, assists with applications, assists with re-certifications and provides advocacy with appeals, denials and other areas relevant to maintaining benefits/resources.
- Initiates and follows up on referrals to agency and community service providers and educates patients on how to access services.
- Develops and maintains effective working relationships with area HIV and social service providers and coordinates activities in order to provide an integrated system of service delivery.
- Documents service delivery and maintains patient records and confidentiality in accordance with JPS Policies & Procedures.
- Attends and participates in agency related meetings, retreats, work sessions, and events as requested.

Qualifications:
Required Qualifications:
- High School Diploma, GED or equivalent.
- Three (3) plus years of experience in customer service environment.
- Must complete all training required of Agency new hires, including any training required by TDSHS HIV Care Services Branch Standards of Care, within established timeframes.
- Successful completion of the HIV Navigation in Texas Learning Series is required within the first year of employment. This includes pre-requisites, learning modules and the face to face Learning Session.
- Successful completion of HIV Prevention in Texas is required within 3 months of employment. (Effective for new hires as of 1/23/2015).
- STD 101: What You Need to Know (online) required of all contractors within three months of employment.
- Bachelor's Degree in a Social Science or related field of study from an accredited University.
- Lived experience as a person affected by HIV, substance abuse, or STIs, and the ability to relate to and understand the challenges faced by clients.
- One (1) year of experience working with a community service program or as an educator.
- One (1) plus year of Project Management Training.
- Community Health Worker Certification.
