Job Summary:
The HIV Patient Navigator supports the Community Health Network’s community HIV program activities by disseminating education and case management assistance to individuals at risk for acquiring HIV and individuals with HIV through early prevention, detection, and linkage to care services. The HIV Patient Navigator builds strong relationships with the community and individuals living with HIV in order to help them stay engaged in prevention, medical care, and adherence to medication regimens.
The HIV Patient Navigator is committed to removing the patients’ barriers to care by identifying critical resources, helping them navigate through health care services and systems, and promoting optimum health. The HIV Patient Navigator must maintain the highest confidentiality regarding patient records and information received in written, oral, graphic, or other tangible forms.
This is a team-based position that reaches into the community through local venues (community groups, religious gatherings, health fairs, etc.), and works collaboratively with clinic and outreach staff. The position will be well informed on the benefits of PrEP, HIV intervention and state (ADAPT), federal and pharmaceutical assistance programs.
Duties and Responsibilities:
- Organize and participate in community health fair events to attract and enroll community members and raise awareness of HIV, PrEP (Post-Exposure Prophylaxis) and how to reduce HIV affection risk
- Support PrEP use and HIV medication adherence by assisting patients enroll in the Texas Department of Health ADAPT Drug Program and pharmaceutical assistance programs such as Ready, Set, Prep
- Provide HIV rapid testing to community members to identify HIV positive individuals and link them into care
- Screen patients for co-existing SDOH needs & referring them into the BrazCo Community SDOH Network to assist with utility, food, clothing, housing, and transportation, etc. is essential to meeting the basic needs that may prevent them from remaining compliant with linkage to care issues
- Serve as primary point of contact for individuals contacted during in-reach, off-site outreach, onsite case management activities including enrollment in Medicaid, Social Security services, public health programs, and navigation through clinical services
- Document and maintain records of encounters with individuals and families
- Provide support and education to assist families when a problem with access or financial assistance arises. May act as family liaison in accessing care

Required Skills/Abilities:
- Proficient in strategies such as motivational interviewing to encourage individuals to start and remain in care
- Contribute to a team-based approach focused on patient care
- Possess the ability to integrate respectfully with individual persons from diverse cultures and income levels
- Valid driver’s license
- Excellent in verbal, written, and communication skills
- Proficiency in basic mathematics and analytical comprehension
- Strong interpersonal skills with a demonstrated ability to work independently on individual and assigned tasks while exhibiting good judgment
Preferred Skills/Abilities:
- Proficiency in spoken and written English and Spanish skills
Required Education and Experience:
- Bachelor’s degree in social work. In lieu of degree, minimum of two years’ experience in HIV in field with patient contact
- High School Diploma or equivalent

- Providing outreach and community education requires performing activities in various locations that include businesses, community events, and varied venues. Position must use personal judgment to assess if there is a danger in conducting outreach, education, and enrollment due to weather, environmental condition, or situations.
