I. Job Summary
The Invasive Services Patient Navigator Nurse is a member of the healthcare team and assists patients' with "navigating" through the healthcare system and get timely care. The Navigator works with the Physicians to coordinate patient care, assist with patient orders, connect patients with resources, and help patients understand the healthcare system while working collaboratively with multidisciplinary and medical teams to deliver quality patient care services. Monitors patient outcomes, and participates in quality improvement activities. Works alongside management staff to implement and develop established or new programs related to the department efficiency and cost-effective program implementation.
Minimum Job Requirements:
Work Experience:
Three years of direct patient care experience in an acute care setting. Previous case management experience or related experience preferred.
License/Registration/Certification:
Current Texas or compact RN license required.
Education and Training:
Bachelor degree in Nursing required. Master degree in Nursing preferred.
Skills:
- Excellent interpersonal communication and negotiation skills.
- Strong analytical, data management and computer skills.
- Current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement.
- Able to work with people of all social, economic, and cultural backgrounds. Culturally sensitive.
- Flexible, open-minded and adaptable to change.
- Understanding of pre-acute and post-acute venues of care and community resources.
- Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.
- Ability to work independently and exercise sound judgment in interactions with physicians, payors and patients/families.
- Bilingual English/Spanish preferred.
- Ability to read and comprehend English at a level necessary to follow written and oral instructions and safety precautions.
