SUMMARY
This position will be responsible for coordinating medical and other health appointments as well as tracking referrals to ensure continuity of care and completion of appointments. The overall goal of this position is to assist in improving the delivery of the health care services that are by UMMA Primary Care Providers. This position requires familiarity with HEDIS and Star Performance Quality Measures.
ESSENTIAL DUTIES
A. Track and monitor select HEDIS and Star Measures.
B. In coordination with primary care provider and patient, assist with scheduling appointments and referrals to external providers and specialists.
C. Ensure the appropriate documentation is obtained from specialist and entered into EHR
D. Develop plan to better improve patient ability to connect with specialty visits and other community resources that are specific to their health care needs.
E. Engage with patients to identify barriers to receiving treatment.
F. Manages and maintains database of high-risk patients and schedules follow-up care.
G. Attends local community meetings related to expanding services available for UMMA patients; represents UMMA at various community events.
H. Performs a wide variety of clerical duties including: preparing routine correspondence, filing, photocopying, faxing, and answering the telephone; and referring patients to appropriate person.
I. Participate in staff and clinical meetings
J. Strictly adhere to universal precautions as established by the Center for Disease Control and Prevention, Occupational Safety and Health Administration, DHS/OA and the Clinic.
K. Submit monthly reports to supervisor and identify opportunities to improve coordination
L. Other duties as assigned
SECONDARY DUTIESAssist coordinating health and resources fairs at Fremont Wellness Center. Engage in outreach activities for the promotion of Fremont Wellness Center.

The environment for this position is medical clinic that is clean and comfortable. It may include some minor annoyances such as noise, odors, drafts, etc. The incumbent is in a non-confined office-type setting in which he or she is free to move about at will. In addition, the incumbent may travel via personal vehicle or public transportation throughout the community.
PHYSICAL ACTIVITYIn the course of performing this work, the incumbent:· Will spend substantial time standing, sitting, speaking and listening
· Must regularly lift and/or move up to 10 pounds, and occasionally lift and/or move up to 25 pounds
· Travel via reliable personal vehicle to other facilities throughout Los Angeles may be required for this position

QUALIFICATIONS/ POSITION REQUIREMENTS

EXPERIENCE / KNOWLEDGE:
To perform effectively in this position, the incumbent must have:
a. 1 or more years experience with programs that support health
b. Working knowledge of HEDIS and Star Quality Improvement measures.
c. Preferred Spanish speaker, ability to read and write
d. Experience with, knowledge of, multi-ethnic and underserved communities
e. Program administration and coordination experience preferred
f. Good oral/written communication in English, interpersonal and oral presentation skills for small and large groups
g. Proficiency with Microsoft Office: Word, Excel, Power Point, and Publisher
h. Must take initiative and work without supervision
EDUCATION:
Degree in Public Health, Community Psychology, Health Sciences or related field
Certification in Health Education, Bachelor’s Degree preferred
