The SNF/ LTC coordinator is responsible for providing time-sensitive administrative support to the SNF and Transitional Care Teams. Through the tracking and close monitoring of all patients admitted to both skilled nursing and long-term care facilities, the SNF / LTC coordinator assists in the Transitional Care team’s mission to improve patient transitions through continuity of care, and to prevent the need for repeat admissions. The SNF/ LTC coordinator is also responsible for proper documentation of all visits for billing and coding purposes, and will collaborate with billing team when necessary.
The SNF coordinator must understand the importance of timeliness and accuracy in ensuring continuity of care, and will communicate with all appropriate parties for admissions, discharges and referrals, and scheduling of SNF discharge appointments. The SNF coordinator will serve as liaison among hospital team, SNF team, admitting facilities as well as PCP offices.
- High school Diploma plus 1-2 years post-secondary education or equivalent work experience.
- Bachelor degree preferred.
- Experience in Microsoft Office Suite of products (Word, Excel, and PowerPoint & Outlook).
- Knowledge of HITS and EMR system.
- Knowledge of Availity.
