The full time Patient Care Coordinator will be responsible for initial patient contact following the patient's first consultation with the physician, educating patients on the appropriate processes and collaborating with other teams to ensure excellent patient care
ESSENTIAL RESPONSIBILITIES:
- Review patient chart and physician notes; evaluate the patient's plan for care.
- Contact patient within two days post physician consultation to introduce yourself and provide a warm welcome to Aspire HFI.
- Track new patients daily and send weekly report to Managing Director.
- Educate patients about the appropriate process, from testing to treatment.
- Collaborate with all financial teams as necessary to ensure financial clearance is obtained for services.
- Facilitate patient connection with any/all staff members for questions or scheduling.
- Improve patient experience and retention while reducing patient cancellation throughout their infertility journey at Aspire HFI.
- Ensure optimal productivity and efficiency.
- Contribute to team effort by maintaining an excellent rapport and working relationship between all Aspire HFI staff other departments within practice and the Prelude network.
- Other duties as assigned.

EDUCATION AND/OR EXPERIENCE:
- High School Diploma or equivalent required
- Minimum of two years of professional experience in a healthcare setting.
