Responsibilities:

• Coordinate overall care through Patient Care life cycle (admission to discharge).
• Assist in recruiting caregiver and nurse to cover all necessary services being provided.
• Coordinate interactions between caregivers/nurses and patients.
• Have a completely understanding of HMO/PPO insurance and help patient understand how insurance works for the patient.
• Ability to call insurance for authorization
• Enter patient/employee information into ABS (database).
• Answer telephone calls for patient intake and other related services.
• Prior experience as medical treatment coordinator is a plus
• Bilingual in plus but not required

Requirements:

• 3-5 years Experience
• High school diploma or Some College
• Computer Proficiency is necessary.
• Valid Driver’s License and auto insurance with a reliable vehicle
• Adhere to HIPPA and maintain client confidentiality
• Mandatory CEU’s
• Negative drug screen
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds

We are strongly considering candidates with experience as a CNA, Home Health Aide, Personal Care Aide/Assistant, or similar positions.
