Job Summary
The Patient Services Rep functions as the Birth Certificate Clerk and is responsible for timely processing and completion of Birth Certificates and required documents related to birth; ensuring documents are completed with the highest level of accuracy, privacy, and in accordance with all applicable laws and regulations.
Licensure and Certification:
- Current BLS provider card
Education:
- Minimum High School graduate or GED equivalent
Experience:
- Minimum 1-2 years of medical office or health information records experience
- Experience in Birth Registration completion preferred
Knowledge and Skills:
- Understanding of HIPAA rules and regulations
- Typing, proper use of language, writing, and telephone skills
- The ability to retrieve, communicate or otherwise present information in various modes

- Frequent sitting and standing/walking with frequent position change.
- Continuous use of bilateral upper extremities in fine motor activities requiring fingering, grasping, and forward reaching between waist and chest level.

- Occasional/intermittent bending, squatting, kneeling, pushing/pulling, twisting, and climbing.
- Occasional/intermittent lifting and carrying objects/equipment weighing up to 25 pounds.
- Continuous use of near vision, hearing and verbal communication skills in handling telephone calls, interacting with customers and co-workers and performing job duties.
