Palliative Care Nurse (Rn) - La/Oc County

American Veterans Hospice is a growing organization with an excellent reputation throughout Orange and Los Angeles counties. We are now seeking PART TIME PALLIATIVE CARE CASE MANAGER to join our team. 

A Palliative Care Nurse cares for patients with life-limiting diseases who are still seeking curative treatments and interventions. The focus is on symptom management, advance directives and other end of life decisions, and education regarding disease processes. The Palliative Care Nurse works collaboratively with a multidisciplinary team including social workers, spiritual counselors and physical/ speech/ occupational therapists. By seeing your patients in the home, you are able to spend time necessary developing relationships, answering questions and assessing the needs of the patient and family on psychosocial, physical, and spiritual level. 

Regular work schedule is Monday through Friday, 8am-5pm. 

Job Requirements: 

• Current California RN License 
• At least one year of professional nursing experience in Palliative Care
• CPR certified (Healthcare Provider)
• Home Health and/ or Hospice experience is a plus
• Current driver's license and clean driving record
• Have and maintain auto insurance coverage
