What You Need to Know
Works under the direction of Palliative Care Director and Program Director, provides nursing care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care. May include care for community-based Palliative and Hospice appropriate patients in the continuum of care.
  • Guided by the interdisciplinary plan of care, gather information related to the patient family physical, psychological, social and spiritual needs, and collaborate with the Palliative Care Team, pt./family, physician, and IDT to coordinate care
  • Initiates and maintains communication with Nurse Practitioner, IDT Team / Palliative Director, and contracted medical groups to coordinate optimal care and use of resources for the patient/family
  • Documents according to protocol and maintains up-to-date patient records so that the care plan's problems, interventions, actions and goals reflect baseline, current, and ongoing status, and changes as they occur within a maximum of 24 hours
  • Participates in Interdisciplinary Team care planning, take vital signs, nursing visits as designated by Palliative Care nursing plan, medication reconciliation, and education on disease process based on patients care plan
  • In keeping with the plan of care, provides education to patient and caregiver(s) as needed based on knowledge needs and learning assessment and patient / family condition
  • Informs the Nurse Practitioner, Palliative Director or Program Director of unusual or potentially problematic patient/family issues
  • Participates in orientation and ongoing education programs and maintains current professional licensure. Demonstrates familiarity and use of agency and state(s) policies and protocols and federal regulations pertaining to provision of hospice and nursing services
  • Participates on committees and special projects and other related duties as assigned
  • May participate in IDT meetings, facility patient conferences, and other patient / family meetings in coordination with IDT and /or Palliative Director
  • Under the direction of an NP or physician, assist in execution of the care plan, including routine and emergent visits
  • Contributes to the on-going care planning process, especially regarding medical needs, with the care team and other agencies as required
  • Gathers information regarding patient clinical status as needed utilizing telephonic, telemedicine, and/or face-to- face appointments
  • Provides support for the Palliative patient and family as new clinical needs are identified or questions arise, with referral to the clinician as appropriate
  • Gathers information regarding the patient for appropriateness for other service lines that will best meet patient needs such as Hospice
  • Collaborate with the rest of the Palliative team including alerting the team to changes and newly identified patient needs as they arise
  • Other duties as assigned

Qualifications
  • Associates degree, preferred
  • Current and valid Licensed Vocational Nurse license in state(s) license(s) where practicing required
  • Current and valid CPR certification preferred, required in Georgia
  • Meet the regulations and requirements of the state(s) in which program provides services
  • Minimum one year of experience working in a licensed field, preferred, required in California
  • Previous experience in hospice, Palliative Care, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing.
  • Must have reliable automobile, valid driver's license, and the minimum state required liability auto insurance
