Hospice Licensed Vocational Nurse (Lvn)

Loyal Way Hospice and Palliative Care, Inc. is seeking a compassionate and experienced Licensed Vocational Nurse (LVN) to join our team. As a per diem LVN, you will provide skilled nursing care and support to our hospice patients and their families under the direction of the DPCS.

Responsibilities:

• Providing direct patient care, including administering medications and treatments as prescribed by the physician
• Assisting with patient assessments and care planning
• Monitoring patient symptoms and providing appropriate interventions
• Collaborating with the interdisciplinary team to ensure comprehensive patient care
• Providing education and support to patients and their families
• Documenting patient care in accordance with hospice policies and procedures

Qualifications:

• Current LVN license in the state of California
• Minimum of 1 year of experience in hospice or palliative care
• Strong assessment and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Flexibility and willingness to work on an as-needed basis
• Valid driver’s license and reliable transportation

Bilingual nurses that speak Spanish are preferred.
