SalaryWe are an established Hospice Agency, which is seeking part time experienced visiting hospice RN. Responsibilities include Home visits, Admissions, Educating, Instructing and supporting the patient, family and caregiver needs. Work closely with other members of the interdisciplinary team to ensure all patient and family care needs are being met. Provide timely documentation of patient services base on plan of care. Identify other physical, psychological and social needs for assigned patients.
Qualifications:
- 1-2 year’s clinical experience in acute setting required.
- Experience with Hospice is preferred.
- Current, unencumbered state license to practice as a Registered Nurse (RN)
- Demonstrated understanding of hospice philosophy
- Professional and compassionate with patients
- Excellent written and verbal communication
- Valid Driver's license, auto insurance and reliable vehicle
- Must be able and willing to travel within assigned territory.
- Multi-lingual is a plus.
