Description
Under the supervision of the CTR Manager, performs assigned duties relating to the implementation and evaluation of the Hyacinth Prevention programs.
Duties include but are not limited to the following:
- Conduct outreach to the hardest to reach YMSM and other high at-risk communities. These communities include women who are substance using, incarcerated, undocumented, uninsured, homeless, teens, or otherwise not aware of, or in denial about, their risk for being HIV-infected. Outreach will include street level and community level outreach in addition to accompanying the testing unit daily to various locations
- Complete the required NJDOH state trainings
- Complete the required HIV testing and counseling certification
- Conduct HIV testing as needed ? Monitor, review and create social media content and activities
- Conduct outreach via dating apps during non-traditional hours
- Focus on getting the Mercer county community educated on STI screenings and treatment
- Assist with following up with positive STI cases and linking them to treatment
- Assist with compiling data for the "Trenton Outreach Project" once rolled out
- Provide relevant literature to clients and members of the community. Engage the community in dialogue pertaining to risk behaviors, risk reduction and testing options for HIV and STIs. Identify obstacles for the provision of services and provide the community with referrals
- Engage in outreach activities at various community sites to identify volunteer community members for potential peer leaders. Assist with delivery of training sessions for peer leaders. Assist and monitor peer leaders with their efforts to arrange and provide prevention education sessions at various sites in the community.
- Actively seek and identify individuals or groups requiring the agency's services.
- Provide risk reduction and health education.
- Serve as a resource for members of the community already infected with HIV to ensure access to needed care and treatment services and to prevent secondary transmission/infection.
- Establish, maintain, and update client case records using approved agency forms and procedures. Ensure confidentiality of all records.
- Prepare monthly reports in accordance with agency policies and procedures.
Professional Responsibilities:
- Attend and participate in staff meetings and supervisory conferences.
- Maintain working relationships with other community providers.
- Observe established administrative procedures.
- Provide written reports and maintain statistics as required.
- Represent the agency at community events, meetings and conferences.
- Attend and participate in staff training opportunities.
Requirements
Qualifications:
- Commitment to Hyacinth philosophy
- Demonstrated ability to work with people from diverse backgrounds and interests.
- Bachelor's degree plus two years experience in HIV/AIDS or related field. A candidate, who has no degree but seems exceptionally suitable, will be considered if he/she has four additional years of relevant experience.
- Proficiency in written and verbal communications.
- Access to currently registered car and a valid driver's license.
