Overview:
Nurse Navigator
Receiving the diagnosis of cancer will often bring up more questions and confusion than answers. Knowing where to even start the journey along the Cancer Care Continuum can be a huge undertaking for a patient. At Willamette Valley Cancer Institute, our OncologyNurse Navigators act as your guides on the initial stage of your journey. As one of the initial points of contact, the OncologyNurse Navigator will help connect the patient with not only the care team in the clinic but provide information and outreach for both patient and family support team members. This position requires not only the tenacity to shift priorities at a moment's notice, but also have the compassionate mindset to put the patient above all else. Experience as a Nurse Navigator is highly preferred.
The general pay scale for this position at WVCI is $38.00-$56.78. The actual hiring rate is dependent on many factors, including but not limited to: prior work experience, education, job/position responsibilities, location, work performance, etc.

Responsibilities:
The Oncology Nurse Navigator is responsible for providing individual assistance to patients, families, and caregivers to overcome health care system barriers and provide timely access to quality healthcare within a community oncology setting. Utilizing the nursing process, the Oncology Nurse Navigator provides patient education and access to relevant internal and external resources. Primary objectives are to facilitate informed decision making and timely access to high quality health and psychosocial care through all phases of the cancer continuum. Routine collaboration with internal departments and external referral resources are anticipated, including scheduling, financial counseling, lab, pharmacy, social work, nursing, surgical, radiation and imaging, nutrition, and others. Regular and frequent interface with practice-based leadership teams, physicians, and clinical staff is required to promote and engage the patient navigation process.

  • Performs coordination of care during the full continuum of cancer care experience
  • Identifies potential and real gaps in care delivery; seeks out opportunities for avoidance and/or closure
  • Prepares for, organizes and participates in the Patient Care Conference (huddle) at regular intervals; communicates interventional outcomes to full care team in a timely manner
  • Performs assessment of patient risk stratification
  • Performs basic psychosocial assessments and provides emotional support; delegates and/or make referrals for additional screening and support as needed
  • Develops appropriate nursing diagnoses and corresponding action plans as needed
  • Supports and/or provides chemotherapy education, adherence and side effect monitoring. Prepare and present education materials to patient and family members. Making sure that the information is both up to date and accessible to all audiences.
  • Assures appropriate consent forms are obtained prior to administration of therapies.
  • Holds or participates in survivorship and advance care planning visits; makes referrals as needed
  • Participates in research, quality, and performance improvement, including reporting on navigation metrics
  • Maintain informational resource binders on available therapies
  • Documents patient teaching in EMR. Communicates all pertinent information to the physician and healthcare team.
  • Triages patient phone calls as needed and documents relevant information in the EMR. Assess patient/family needs and provides teaching and other instructional material. Demonstrates awareness of community resources available to patients and assists with referrals as necessary.
  • Supports patient advocacy and empowerment
  • Knowledgeable of practice operations guidelines, practice organization and healthcare economics
  • Participates in Team Meetings as part of the Support Services Team. Works collaboratively with all members of the multidisciplinary team.
  • Maintains patient confidentiality.
Qualifications:
  • Active Oregon State RN license
  • BSN and Oncology Certification (OCN) preferred
  • 5 (five) years of experience as a Registered Nurse in an oncology practice or applicable acute care environment
  • 1 (one) year experience as a Nurse Navigator or Case Management; Nurse Navigator Certification preferred
  • Demonstrates leadership, interpersonal and collaboration skills
  • Ability to apply the full nursing process in day-to-day clinical operations, including assessment, analysis and nursing diagnosis, care planning, implementation and outcomes evaluation
  • Ability to assess health disparities, identify needs of populations and cultures found within the general facility service area, describe a patient navigation process to address those needs, and document outcomes
  • Ability to prioritize and re-prioritize quickly
  • Strong organizational and critical thinking skills
  • Ability to apply and/or oversee care coordination in conjunction with the multidisciplinary team
  • Strong verbal and written communication skills
  • Working computer and internet skills, including an electronic EMR system
  • Basic knowledge of local community resources
