Job Description

Position Summary:
The Nurse Navigator is responsible for performing nursing assessments and triage of patient care needs for new and ongoing patients, including patients on a clinical research study. Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients. The position will work side-by-side support a provider daily.

Essential Job functions:

  • Timely navigation of patients across the continuum of care beginning with diagnostics through cancer survivorship, coordinating care with staff and providers in other clinical departments, Genetic Counseling and Cognitive Therapy, and refers to community resources to facilitate timely diagnosis, treatment and survivorship.
  • Assesses, plans, and provides nursing care, counseling and education to cancer patients providing physical, psychological, and emotional support to patients and their family members.
  • Responsible for providing telephone triage, advice and health education.
  • Answer patient calls on a wide range of topics, asks the right questions to get the information needed to make an accurate assessment.
  • Ability to assess the problem, based upon the conversation, and use clinical judgment, knowledge and experience to direct the conversation and guide in the decision to give self-care advice, refer the patient to the oncologists, make an appointment or call 911 for emergency assistance.
  • Delivers direct patient care within the scope of licensure and in accordance with SUMMIT policy.
  • Communicates with physicians and other members of the interdisciplinary team, within SUMMIT, as well as with outside providers.
  • Facilitates scheduling of appointments with staff to ensure timely referral and transition into specialty services.
  • Documents all patient visits and communication in electronic medical record. Manages and submits statistical data monthly.
  • Develops an expertise in the chemotherapy protocols to support patients during chemotherapy treatment.
  • In depth knowledge and understanding of disease processes, drug reactions and preventive care.
  • Other duties as assigned.

General Job functions:

  • Participates in community outreach programs. (Cancer Support Group Meetings, breast cancer patient classes, evening lecture programs).
  • Coordinates the availability of patient education materials as well as being familiar with resources available in the community.
  • Assists Clinical Manager with the development and implementation of Cancer Care Programs in multiple areas (e.g. survivorship, patient education materials, identification of community resources, marketing, prioritizing work flow).

Education, Certification, Computer and Training Requirements:

  • Bachelor’s Degree required.
  • State of Oregon Registered RN license.
  • OCN preferred.
  • ONS Chemotherapy Biotherapy Certification within 90 days of hire/transfer
  • 3 years’ oncology experience preferred.
  • BLS Certification required.
  • 5-8 years of experience preferred.
  • Ability to communicate in English, both orally and in writing.
  • Ability to use Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred.
  • Ability to use Standard Office Technology in a Window based environment preferred.
  • Experience with Epic preferred.

Physical Job Requirements:

  • Physical mobility, which includes movement from place to place on the job, taking distance and speed into account.
  • Physical agility, which includes ability to maneuver body while in place.
  • Dexterity of hands and fingers.
  • Endurance (e.g. continuous typing, prolonged standing/bending, walking).

Travel:

  • Travel to satellite locations for meetings

Schedule will be Tuesday -Friday. Either 4 10's or 4 8's
