This is a full-time, benefit earning position. This position is internally classified as Case Management Assistant.

In this position, you will:
- Assess, plan, and coordinate patient care according to established clinic policies and procedures, objectives, performance improvement activities and safety, as well as environment and infection control standards.
- Manage the Hypertension Management Program, to include but not limited to referrals to the program, outreach to patients and scheduling to the group sessions; set up and breakdown of group sessions, managing patient flow during the session.
- Conduct outreach to patients with high blood pressure to ensure optimal care outcome and scheduling follow-up visits with their primary care provider.
- Facilitate collaboration between community health educational resources and the patients to meet the patients' health care needs.
- Identify clinic and community resources and coordinate appropriate referrals.
- Document and monitor patient care visits in EPIC, the electronic medical record.
- Maintain accurate records and ensures confidentiality of all data collected during health promotion and prevention services.
- One of the following:
- Two years or more of responsible, paid social service experience providing direct client service.
- A two-year human services certificate.
- Two years or more of college coursework in the behavioral sciences.
- Ability to communicate fluently in Spanish and/or Hmong in addition to English, both verbally and in writing.
- Experience:
- Using computer programs such as EPIC to perform data entry where accuracy is essential.
- Working with multiple providers, staff and collaborative partners to improve the care to diverse and underserved people with a variety of cultural and language barriers.
- Knowledge of community resources and social services available to clients.
- Ability to:
- Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
- Communicate effectively both in writing and verbally.
