Social Worker-Housing Case Management

Minnesota Assistance Council for Veterans (MACV) is a statewide nonprofit organization with a mission to end Veteran homelessness in Minnesota. MACV provides comprehensive programs in the areas of housing, employment, and legal services to support Veterans and their families throughout Minnesota who are homeless or at risk of homelessness. MACV operates a budget over $14 million annually and has a team of approximately 100 employees throughout Minnesota. The organization has office locations throughout the state, with administrative functions centralized within a headquarters office based in St. Paul.

Position Description:

This Licensed Social Worker position is part of the Minnesota Veterans Entering Stable Tenancy (MNVEST) Subsidy program that is operated with State of Minnesota funding by MACV. MNVEST provides a rental subsidy and clinical Case Management services to Veteran households experiencing homelessness. This position will provide intensive Case Management services to Veterans and their families receiving vouchers to exit homelessness.

Primary Duties and Responsibilities:

• Perform intakes that include required documentation for admission, referral needs, and explanation of program and expectation.
• Develop a housing stability plan with the Veteran/household which includes short-term and long-term goals with objectives and update case plan as required.
• Provide virtual and in-person Case Management from a harm-reduction, strength-based model.
• Provide financial assistance in accordance with voucher program policies and procedures.
• Develop a discharge plan with participants addressing permanent housing stability, economic stability, emotional stability and self-determination.
• Assess Veterans and connect participants to necessary services by coordinating with other service providers to ensure access, eligibility and follow-through by participant and provider.
• Conduct ongoing evaluations of the participant’s progress through the plan.
• Maintain strong working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available.
• Work with other program staff to ensure compliance with program outcomes and requirements.
• Work with partner agencies to review eligibility and support program implementation to include monitoring visits and quality control.
• Use database to track participant and program progress, maintain up-to-date case files and case notes, and ensure programmatic compliance.

• Strong communication skills to communicate directly with Veterans and community providers
• Strong problem-solving skills and ability to work under tight timeline toward crisis resolution for Veterans seeking assistance in crisis situations
• Two plus years’ experience of working with vouchers and/or subsidized housing and in navigating permanent subsidy programs, or in housing Case Management working with subsidies and waitlists preferred.
• Experience working with individuals who are homeless, chemically dependent or have a mental illness desired.
• Veteran, or knowledge of veteran issues, preferred.

Salary Range:

BSW/LSW: 52,000 - 60,000

MSW/LGSW: 55,000 - 63,000

MSW/LISW, LICSW: 60,000 - 68,000
