Diamond Willow is an Assisted Living and Memory Care facility that started in 2004. Our facility in Proctor, MN consists 5 cottages/suites and up to 56 residents. We provide the utmost quality of care with an emphasis on person-centered care planning in our home like atmosphere. We believe our healthcare workers are the foundation to our success.
Responsibilities
Observing, tending to, and documenting resident's health needs including appts., labs, etc.
Communicating with Administrator & Clinical Supervisor regarding relevant staff and resident information and/or concerns
Completing Vulnerable Adults, MAARC Reports, and Incident Reports
Working with Clinical Supervisor to ensure MD orders are processed and performed
Updating families on any changes to the resident and addressing family or patient concerns.
Providing tours for potential admits and admitting new residents.
Addressing and documenting any staff issues that arise.
Overseeing staff and ensuring all staff are meeting job expectations and requirements.
Requirements
Limited on-call responsibilities.
Must be able to occasionally fill in aide shift when needed.
Maintaining confidentiality and professionalism at all times
Must be able to work as a team member and maintain a positive demeanor
Qualifications
Graduate from an approved Registered Nurse (RN) program
Possess a valid Minnesota Registered Nurse (RN) license
Minimal of 1 year experience in long-term care facility preferred
