About You
We are seeking someone who will partner with the client development department, residences, and outpatient clinics. Responsibilities include facilitating tours, reaching out to hospitals and referents, coordinating and completing assessments. Collecting and understanding admission documentation as well as CCL charts.
You will need:
• Full Vaccination against COVID-19 – CA required
• Bachelors degree (health care administration, nursing, public administration, psychology)
• Five years of professional behavioral health care experience
• LVN/LPT certification - required

Your Responsibilities
• Experienced in review referral packets of possible candidates for admission to Psynergy Programs system of care and support, set up a time to see client as soon as possible.
• Ensure as an organization we maintain a full census and maximize capacity across all programs
• Travels to various locations to interview potential clients and determine readiness for a community placement.
• Completes all pre-admission forms, intake, and Community Care Licensing (CCL) documents as required by CCL.
• Collects clinical records and other required documentation in accordance with agency, payer and regulatory requirements utilizing paper or computer technology.
• Submits appropriate paperwork to management such as expense reports and call reports in accordance with client development procedures.
• Assists and resolves customer complaints in a timely manner, notifies management of problems and suggests collaborative solutions.
• Review and present information to CD Manager and other members of the behavioral health team about client’s family and social background pertinent to diagnosis and treatment.
• Applies technical knowledge of agency programs and services to best meet customer needs.
• Understands and supports agency services to provide appropriate and effective service to all clients, family, county partners, community members, and fosters the development of long-term relationships.
• Attends and participates in staff program, agency and community meetings and training as required.
• Promotes a professional image and positive attitude in all interactions with the public.
