Mds Coordinator / Utilization Review- Lpn/Rn - Riverdale Post Acute

We are currently looking for an experienced MDS Coordinator - LPN to join our team at Riverdale Post Acute.

MDS Coordinator- LPN - Full Time - Monday - Friday

• Collects MDS documents within mandated, established and expected time frames.
• Compares data entered against the original MDS document; makes corrections as needed.
• Files the MDS documents in residents' charts.
• Maintains confidentiality of all patient care information.

Requirements:

• Current LPN License
• Experience as a MDS Coordinator in skilled nursing facility
