Description
Danish Care Center is a 65-bed skilled nursing facility focused on the well-being and happiness of our residents.
Danish Care Center is looking for a LVN with MDS experience to join our team.
Responsibilities of the MDS Nurse:
- Oversee utilization of interdisciplinary rehab team process in the formulation of MDS/RAI's.
- Observe direct nursing care, review documentation, and report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Nurse Supervisor
- Plan, schedule and revise the MDS, including the implementation of RAPs, triggers, and care plans
- Complete the entire MDS process is in a timely and consistent manner.
- Ensure the successful, timely electric transmission of the MDS package.

- Must have excellent leadership and communication skills;
- Must be a licensed LVN in the state of California;
- Must have skilled nursing experience;
- Must have basic MDS experience.
