SalaryFull Time (80 hours/2 weeks) Mon-Fri 8:30am-5pm
- Duties include case management for an assigned group of Assisted Living and Basic Care residents, including effective communication with residents and resident families.
- Provides effective direction, feedback and support to Medication Aides and CNAs caring for Towers residents.
-Assists with quality assurance/improvement, including evaluation of staff and resident satisfaction surveys, development implementation of improvement strategies, development of positive resident relations, and effective resolution of resident concerns.
-Supervises medication administration/assistance program. Manages medications and treatments as ordered by the physician. Assures proper storage of resident's medications. Monitors & supervises MA duties with regard to medication reminder/assistance program.
-On call for Housing & Assisted Living department,rotating with Housing AL nurse leadership team once every 5 weeks.
- Must be flexible with start and end times of shifts based on case management needs of residents.
- Requires independent judgement and knowledge of department and facility procedures
Reports to Senior Executive Housing & Assisted Living and Director of Nursing Housing and Assisted Living.
- Must have LPN License with at least 1 year experience
