Responsibilities:
- Provide compassionate and comprehensive care to hospice patients
- Conduct assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor and evaluate patient's condition and response to treatment
- Collaborate with interdisciplinary team members to ensure holistic care
- Educate patients and their families on disease management and end-of-life care options
- Document patient care activities accurately and timely
Experience:
- Minimum of 1 years of experience as a LPN in a clinical setting
- Experience in hospice or palliative care is highly preferred
- Proficient in medication administration and management
- Familiarity with medical terminology, medical records, and documentation practices
- Knowledge of Durable Medical Equipment (DME) utilization and coordination
- Experience working with patients of all ages, including children, is a plus
Note: Experience with ER, Sonography, Athenahealth, or Botox is not directly relevant to the role of a Hospice Nurse. However, if the candidate has experience in these areas, it may demonstrate their versatility and adaptability.
