Job title: Hospice Registered Nurse

Job Summary:

We are seeking a compassionate and dedicated Hospice Registered Nurse to provide end-of-life care and support to patients and their families. The Hospice RN will work closely with a multidisciplinary team to coordinate care, manage symptoms, and ensure the comfort and dignity of patients in their final days.

Responsibilities and Duties:

• Conduct thorough assessments of patients' physical, emotional, and psychosocial needs
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Monitor and manage symptoms, provide pain management, and promote comfort measures
• Educate patients and families on end-of-life care, advance directives, and available resources
• Provide emotional support and guidance to patients and families throughout the hospice journey
• Maintain accurate and timely documentation of all care provided
• Collaborate with physicians, social workers, chaplains, and other healthcare professionals to ensure holistic care

Qualifications and Skills:

• Current RN license in the state of practice
• Minimum of 2 years of nursing experience, preferably in hospice or palliative care
• Excellent communication and interpersonal skills
• Strong critical thinking and problem-solving abilities
• Compassionate and empathetic attitude towards patients and families
• Ability to work effectively in a fast-paced and emotionally demanding environment
• Knowledge of end-of-life care principles and practices
• BLS certification
