THIS IS AN ON-CALL MONDAY - FRIDAY NIGHTS POSITION

This nurse owned and run hospice provides a culture of autonomy and philosophy that recognizes the individual needs of our patients and caregivers. This LPN is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Job description
- Provides direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end of life care
- Implements current nursing practice following a comprehensive assessment and the plan of care.
- Provide accurate and timely documentation of patient services to reflect the plan of care.
- Participate in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
- Provides appropriate pain and symptom management
- Performs other duties as assigned by the registered nurse.
Essential Duties
- Prolonged or considerable walking or standing.
- Able to lift, position, and/or transfer patients.
- Able to lift supplies and equipment.
- Considerable reaching, stooping, bending, kneeling, and/or crouching.
- Visual acuity and hearing to perform required nursing skills.
- Available for rotating on-call schedule with team members
- Maintains a current CPR certification.
- Must be able to drive within a 65 mile radius.
- Must be able to bend, twist, sit and stand for long periods of time, must be able to lift 50 lbs without assistance
