What will you be doing in this role?
Our Case Managers conduct case management on patients in the inpatient, intensive outpatient, residential care and Skilled Nursing Facility settings. CSMNS has developed a case management program that is geared towards the management of patients while hospitalized and upon discharge from various care settings. Using the Case Management Society of America's (CMSA) model, we describe our approach as "a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs through communication and available resources to ensure quality cost-effective outcomes."
As a Lead Case Manager, you will assist in the daily operations of the inpatient case management department.
Summary of Essential Duties:
- Conduct team audits
- Respond to sick calls and unscheduled absences and adjust staffing accordingly
- Arrange and adjust contingency schedules for vacations, unscheduled days off, staffing crises
- Responsible for holiday/Weekend CM schedules
- Function as the contact person for clinical issues and workflow processes for staff
- Handle responses to inpatient stay denials
- Monitor inpatient census to ensure that patients are correctly classified
- Monitor 30-day readmissions
- Assist with training new hires
- Participate in projects and initiatives as approved by the manager or director
- Provide oversight to colleagues to ensure assistance is provided when needed
- Function as a triage/resource to MDN Inpatient Case Management operations.
Education:
- Graduate of an accredited nursing program required
License/Certifications:
- Current, unrestricted California RN or LVN license required
- BLS from the American Heart Association or American Red Cross required
Experience:
- A minimum of 3 years of acute care nursing experience required
- At least 3 years of Utilization Review, Case Management, Discharge Planning and HMO experience preferred
