Under the direct supervision of the Family Support Services (FSS) Senior Program Manager, The FSS Lead Case Managerprimary responsibilities will be the following: Identify and provide training to new and existing Case Management staff. Work closely with the Senior Program Manager, and the QA Coordinator to ensure the FSS program is meeting and exceeding deliverables and providing high quality care to families enrolled in the program and support program enhancement. Collaborate in the oversight of program referrals, and case assignment. Partners with multi-disciplinary care teams within TVHC with the intent on providing integrated healthcare services to FSS clients while supporting the case management team with service delivery and day to day operations.
Family Support Services is comprised of three programs – Targeted Case Management, Cal Learn and Alameda County Public Health Home Visiting programs. The service area for this program is Southern Alameda County comprised of Hayward, and the Tri-Cities (Union City, Hayward, Castro Valley, San Leandro/San Lorenzo, Livermore, Newark and Fremont).

Compensation: $32 - $35 per hour, depending on experience.
Responsibilities:
The Lead Case Manager will provide comprehensive case management services to up to 12 enrolled families (50% of the caseload of a full-time FSS Case Manager) and administer health screens and psychosocial assessments, engage in child development activities and parenting education with clients, develop and coordinate individual service plans. Services are conducted in clients homes, TVHC clinics and in the community.
- Coordinate onboarding/offboarding schedules for FSS employees
- Provide staff training to include all program scopes of work.
- Assist with quality assurance activities to ensure program compliance.
- Oversight of ongoing trainings, program document management and workflows.
- In collaboration with the Senior Program Manager, assist and lead topics in the monthly staff meetings.
- Provide supervisory support to the case management team as needed.
- Participate in the hiring of new program staff.
- In collaboration with the Senior Program Manager and QA Specialist support program annual peer chart audits.
- Oversight of the program waitlist and support the Senior Program Manager with new case assignments for Case Managers.
- Provide advocacy support (eyes and ears) for the case management team, the Senior program manager and Director of Care Coordination.
- Oversight of the Case Management program supplies (diapers, clothes and educational materials).
- Seek and assist with applying for grants to secure program supplies as needed. (i.e. Baby to Baby, HAMO)
- Take lead in the coordination of ongoing program events; team meetings and client events.
Direct Service
- Provide case management services to up to 12 families each month and adhere to visit standards defined in contract scopes of work.
- Develop individual service plans with each client to meet the health, psychosocial, educational, vocational and financial needs of pregnant and parenting teens, adults and their families.
- Monitor client progress to determine effectiveness of service delivery and to assess progress towards individual goals and objectives.
- Conduct psychosocial assessments with pregnant and parenting clients.
- Engage In individual and group supervision by presenting client cases to assess needs and develop interventions.
- Provides supportive counseling as needed and assess and make all appropriate internal and community referrals.
- Engage clients in twice a month sessions at minimum.
- Conduct home visits with clients to maintain communication and assess the adolescent and their children’s living environment.
- Maintain client information and case documentation in a confidential and professional manner.
- Link clients to the school system for enrollment into high school or continuation programs, junior high school or post-secondary education/vocational programs.
- May facilitate parenting groups at local school site.
- Assess the entitlement of childcare, transportation and ancillary services for Cal Learn clients.
- Provide clients with knowledge about pregnancy, child development, parenting, nutrition, family planning and educational options.
- Patriciate in bi-annual peer chart audits for quality assurance monitoring.
- Participate in maintaining a network of services and resources for clients in Southern Alameda County.
- Assist with outreach services to identify and bring in eligible clients into the program.
- Participate in internal and community Trainings.
- Perform other duties as may be required.
Requirements
- Bilingual in English/Spanish required.
- Bachelor’s degree in social work, psychology preferred.
- Associates degree required, or minimum of two years of case management experience in a health and human services field required.
- Previous experience working with adolescent population required.
- Ability to travel to various site locations as directed by the program.
