Description

Position Title: Transitional-Age Youth Lead Case Manager

Reports To: Leadership &STAY FLY Programs Manager

Community: San Mateo County

Office Location: Redwood City, CA (up to 50% hybrid role of in-person and remote)

Compensation: $27.01/hour (approx. $56,180/year), generous benefits package

Status: Full-time, non-exempt 

 

Program Summary

The STAY FLY Program builds agency, self-sufficiency, and self-advocacy in transitional-age young adults between the ages of 18-25 while decreasing their likelihood of deeper justice system involvement. 

 

Transitional-Age Youth Lead Case Manager - Position Summary

The Transitional-Age Youth Lead Case Manager will offer structured support and guidance to justice system-impacted transitional-age youth (TAY) between the ages of 18-25. In addition to facilitating legal education workshops, the Transitional-Age Youth Lead Case ManagerCase Manager work with young adults to set goals around education, employment, and building life skills in order to prevent future system involvement. The Lead Case Manager is additionally responsible for supporting the planning and execution of pro-social and civic engagement events for young adults, as well as engaging in community meetings and events that serve FLY’s youth population.  

 

Position Responsibilities

 

Provide Case Management, Coaching, and Advocacy for Young Adults

  • Provide case management and coaching for 2-3 hours per month depending on the program phase for a caseload of up to 15 young adults
  • Develop individualized development plans to support young adults in building social emotional and self sufficiency skills to achieve goals around education, employment, decreased justice system involvement etc.
  • Respond to young adult crises and basic needs supports utilizing community services and resources
  • Support young adults with systems navigation by attending education meetings, court hearings, preparing court reports, and engaging in community partner meetings
  • Document all client interactions including case notes and case management progress into internal tracking database, Efforts to Outcomes (ETO), and maintain thorough case management paperwork in client file
  • Compile relevant data for program funding sources related to young adult and/or program successes, highlights, or challenges

Facilitate Law-Related Education Workshops

  • Lead the coordination and facilitation of weekly law-related education workshops and maintain positive relationships with young adults participating in the workshops
  • Administer necessary program evaluations and surveys at the beginning and end of workshop cycles
  • Incorporate strategies into weekly workshop facilitation for recruiting young adults into case management
  • Provide additional resources to young adults who participate in the workshops, but may not be eligible for case management services
  • Lead curriculum training sessions for staff and volunteer facilitators, as needed. 

Facilitate and Plan Program Activities in Collaboration with Program Team

  • Execute group events for young adults who participate in the program via the caseload or workshops in collaboration with the Program Coordinator
  • Provide updates to young adults on caseload about upcoming events and collaborate with the program team to ensure attendance at group events
  • Provide and coordinate transportation for young adults to and from activities and events

 

Collaborate and Engage with Education, Community, and System Stakeholders 

  • Participate and engage in education, community, and systems meetings/events to understand the needs of the community in which the young adults live
  • Maintain positive relationships with education, community, and system providers, including probation officers and community-based organizations
  • Lead efforts supporting young adults in participating in community spaces that allow them to elevate their voice, engage in advocacy, celebrate their accomplishments, and take action on issues impacting young adults and the community
  • Support the Programs Manager with external presentations about the STAY FLY Program and represent the agency at community meetings, as needed

Other duties as assigned and needed, which include supervision responsibilities. 

 

Compensation & Benefits

This position is a full-time non-exempt role at a rate of $27.01/hour with differential pay options based on an advanced degree and, when applicable to the role, speaking another language. Our Total Rewards Package also includes the following:
