Job Description:
Responsibilities:
1. Client Assessment: Conduct thorough assessments of potential clients to determine their eligibility and care needs.
2. Program Eligibility: Review and verify documentation to confirm eligibility criteria for enrollment in the Source and CCSP programs.
3. Intake Process: Facilitate the intake process by collecting and documenting client information, including medical history, care preferences, and personal details.
4. Communication and Collaboration: Collaborate with healthcare professionals, case managers, and family members to gather additional information, address concerns, and ensure a smooth transition for clients.
5. Care Planning: Collaborate with the care coordination team to develop personalized care plans based on clients' needs, ensuring all services required are accurately identified and scheduled.
6. Documentation and Record Keeping: Maintain accurate and up-to-date records of client information, assessment findings, and communication for reference and reporting purposes.
7. Program Liaison: Serve as a point of contact for clients, addressing inquiries, providing program information, and guiding them through the enrollment process.
8. Compliance and Regulations: Stay updated on program guidelines, policies, and regulatory requirements to ensure full compliance during the intake process.
9. Continuous Improvement: Contribute to ongoing process improvement initiatives to enhance the efficiency and effectiveness of the intake process.
10. Act as ongoing liaison between caregiver and client
11. Responsible for ongoing tracking of onboarding process per company guidelines
12. Collection and analysis of all required documentation for caregiver compliance through onboarding process
13. Maintaining constant, consistent contact with caregiver and client
14. Ensure all new hires comply with company, Federal, and State regulations required for employment
15. Create profiles in various systems to ensure timely and accurate information is stored and tracked throughout the onboarding process
16. Perform employment eligibility and verification, including: background checks, I-9 completion, etc.
17. Gather and process employee file paperwork and maintain applicant profiles across systems
18. Maintain confidential records (office employee files, health records, protected information)
Qualifications:
1. Education: Preferred - Bachelor's degree in healthcare administration, social work, nursing, or a related field.
2. Experience: Previous experience in healthcare intake, case management, or a similar role is highly desirable.
3. Knowledge: Familiarity with Medicaid Waiver programs and their eligibility criteria is advantageous.
4. Communication Skills: Excellent verbal and written communication skills to effectively interact with clients, healthcare professionals, and internal teams.
5. Attention to Detail: Strong organizational skills with a keen eye for detail to accurately collect and record client information.
6. Empathy and Compassion: Demonstrated ability to empathize with clients and their families while maintaining professionalism and confidentiality.
7. Adaptability: Ability to thrive in a fast-paced environment and handle multiple tasks simultaneously while maintaining a positive attitude.
8. Problem-Solving: Strong critical thinking and problem-solving skills to assess client needs and provide appropriate solutions.
9. Technology Proficiency: Proficient in using electronic health records (EHR) systems and MS Office suite.
