SalaryWe are looking for a self-motivated, patient-focused intake coordinator to join our home health and hospice team! The primary focus would be for our home health intake team but this role may help with some hospice intake as well.
Responsibilities:
Ensures the Referrals made by the providers are processed to ensure patients receive timely follow-up on their diagnosis.
Schedule aides and nurse visits within EMR system
Qualifications:
- Experience with home health/hospice and/or healthcare - preferred
- Experience with the intake process and procedures - required
- Experience in verifying insurance, Medicaid and Medicare – required
- Experience with Microsoft Office (Excel, Word) and Microsoft Outlook – required
- Kinnser experience - preferred
- Detail oriented and highly organized
- Effective written and verbal communication
