The purpose of this position is to efficiently and timely manage the referral process for Touchstone Communities. By possessing a comprehensive understanding of the admissions process and qualifications for admission, the Central Intake Coordinator will effectively manage the workflow of the referral to successfully place patients in the appropriate Touchstone Community where they will have appropriate care and positive outcomes and to create a professional first impression of Touchstone to our customers.
- Manage the timely response of all referrals for the enterprise as a whole.
- Effectively communicate with team members so that timely notification of referrals is met.
- Gather sufficient pre-admission data to determine resident level of care and the ability of the Community to properly care for the resident.
- Coordinate assurance of clinical capabilities appropriate for each referral with community of choice to meet customer’s needs in a timely manner as determined by leadership of Touchstone.
- Obtain benefit verification and then perform timely authorization, when necessary, once benefit verification has been made.
- Assume accountability for creating waitlist in Point Click Care, the CRM module and reporting as directed by supervisor.
- Must possess a valid nursing license that is in good standing with the State, (LVN or RN) with 5 years' experience in long term care case management and admissions.
- Technology expert in Microsoft Office, billing portals, managed care portals and experience using PCC.
