Intake Coordinator

Job Summary

Primary function is to facilitate the referral intake process, coordinate care with the interdisciplinary team, the patient/client, their family, and the referral source.

Essential Functions

• Receives and appropriately documents new referral in a timely manner
• Participates in coordinating care with Nursing Supervisors, patients/clients, and the interdisciplinary team
• Provides effective communication to patients/clients, staff members, other health care professionals and referral sources
• Responds to Agency and patient/.client needs in a professional and creative manner
• Demonstrates commitment, professional growth and competency
• Participates in Agency-sponsored in-service training
• Participates in the Agency's QAPI program
• Promotes the Agency philosophy and administrative policies to ensure quality of care
