REPORTS TO: Director of Marketing
ROLE DESCRIPTION: The Intake Coordinator works in conjunction with the marketing outreach team and the scheduling staff to ensure that all referrals are converted into active census timely.
RESPONSIBILITIES:
- Coordinate all daily patient referral and intake operations, including enrollment of patients and interaction with the sales team, clinicians and physicians, and their offices.
- Manage the efficient patient enrollment process by:
- verifying patient and insurance eligibility,
- setting-up patients’ information into HHA Exchange,
- facilitating face-to-face physician visits from Maximus and Insurance Plans.
- Follow up with Doctors’ Order (M11Q) and make sure of the timely completion
- Manage ongoing interaction with patients to educate them on the services available, reduce fatigue, and to ensure maximum impact on the sales pipeline.
- Accompany and support the marketing and outreach team in their street and event marketing efforts.
- Maintain relationships with insurance partners, obtaining regular status updates on cases referred to these insurance plans.
- Follow up each patient’s status from the date of inquiry through enrollment for long-term care, keeping the patient, and family members informed at all times.
- Ensure that all compliance stipulations are followed, i.e., all state, federal, and referral/intake regulatory requirements for home health.
- Support the implementation of improved work methods and procedures to ensure patients are admitted in care timely.
- Ensure the seamless transition of patients to home care by providing preparation for home care, plan of care initiation, and coordination of care with multiple service providers and agency team.
- Maintain comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by the Agency.
QUALIFICATIONS:
- Strong oral and written communication and interpersonal skills.
- Strong organizational skills.
- Ideal candidates will possess previous home health experience with the referral process, including insurance verifications.
- At least two-year experience in the following areas: marketing, administrative support and working with the geriatric population
- Ability to multi-task and prioritize workflow and use time efficiently
- Ability to show strong attention to detail and follow through to resolve issues and discrepancies
- Bilingual a plus (English, Spanish, French/Creole)
- HHA Exchange, 1 year (Preferred)
- Previous Home Care experience, 1 year (Preferred)
- Associates degree or Bachelor’s degree (Preferred)
