We are looking for a Weekend Intake Coordinator to join our Home Care team. We will consider either an LPN or RN for this position.
Schedule: Saturday & Sunday 8, 10 or 12 hours per day
Responsibilities:
- Screen, triage, and process requests for agency services inclusive of admission criteria, discipline and programs, utilizing and demonstrating a solid understanding of home care criteria, and payment sources.
- Assess the appropriateness of each referral according to Agency policies and procedures and in collaboration with the primary physician and referral sources.
- Utilize all computerized referral source programs and reporting programs timely and effectively.
- Facilitate and ensure quality, coordinated and timely clinical services as the patient moves across the continuum of care.
- Ensure all pertinent data is accurate and complete, utilizes all available information and resources to promote quality care and the achievement of patient goals and outcomes.
- Demonstrate knowledge of home care processes in all communications with referral sources.
- Work collaboratively with scheduling, billing and quality departments to ensure and facilitate the home care services are in compliance will all regulatory and agency guidelines.
- Communicate effectively with all internal and external contacts utilizing excellence in customer service skills.
- May attend appropriate agency and community meetings as assigned and directed.
- Will perform other duties as assigned.
Qualifications:
- Must be a graduate of an accredited LPN or RN program.
- Current LPN or RN license and maintenance of license in the state of New York.
- Minimum of 1 year of clinical nursing experience.
- Home care experience preferred.
- Previous chart audit/review experience preferred.
- Proficient with Microsoft Office suite.
- Must be a team player, with excellent communication, interpersonal, and customer service skills.
