Queens Centers for Progress, a human services agency providing person-centered services for children and adults who have intellectual and developmental disabilities, is seeking an Intake and Curriculum Coordinator for our Bellerose Day Program located in Bellerose, Queens, NY.
Summary of position: To facilitate DH intakes into program and develop a DH training curriculum.
DUTIES:
The Intake and Curriculum Coordinator is responsible for Intake and screening into program: Meet with applicants and provide a tour of program. Ensure admission paperwork is complete, i.e. LP, LOC and OPW authorization and approvals. Facilitate intake meetings and enter new admissions and discharges into Choices database, paperwork into Precision Care and check medical through E-paces. Attend monthly QCP Intake Committee meetings, aging out fairs and monthly DD Council meetings. Develop and create a DH program curriculum for small groups in program rooms and teach DSP's how to implement and maintain an active curriculum and schedule throughout the day in program rooms. Facilitate meetings with Supervisor and DH Specialists to develop creative programming. As assigned by Director/Assistant Director, assist group rooms with daily activities. If needed, assist Supervisors in the completion of daily, monthly and semi-annual documentation.
Minimum Requirements: Bachelor's degree in Human Services Field such as psychology, social work, education, etc. from accredited University and experience in the field required.
