Job Purpose
The Intake Coordinator oversees the interest or referral of potential clients to all JESPY programs and acts as the primary liaison between the potential client/family and JESPY. The Intake Coordinator is the first face of JESPY and provides general information regarding all programming. This position ensures that the potential client has been screened and appropriately documented to begin consideration for JESPY programming. The Intake Coordinator is also responsible for completion of all required documents received and forwards in a timely manner to Managers, Supervisors, Coordinators, Clinicians and Staff for Intake the review process and scheduling of Intake interviews/orientation. This position will serve as the agency liaison to the families and may provide case management services throughout the referral and Intake process during the orientation period. The Intake Coordinator will cooperate with the licensee and Department of Human Services, as well as other State Agencies, in any inspection, inquiry or investigation. During the intake process, the Intake Coordinator acts as the agency benefits & entitlement specialist and is responsible for assisting clients with the acquisition of Social Security benefits (SSI/SSDI), Medicaid, etc.
Essential Duties & Responsibilities
- Actively promotes client intake to the organization to build/maintain census; initiates and follows through on each step of the intake process
- Review intake information for all interested parties regarding all JESPY programs
- Inform parents of the various funding or entitlements which may be available, i.e., DDD, Social Security, Medicare, etc.
- Promotes JESPY programming throughout the agency ensuring collaboration in all areas of the intake process
- Communicates and cooperates with Support Coordinators, DDD entities, and other social service agencies as required.
- Provides support to families and relevant provider agencies throughout the Intake process
- Ensures eligibility to care, including Tier appropriateness for DDD referrals
- Assist Lead Intake Team in assessing all potential IDD referrals regarding acceptance or denials while identifying appropriateness of placement in programs and examining barriers to admission
- Educate clients, parents or guardians of their financial responsibility depending upon individual financial funding and or private payment
- Upon acceptance, collaborates with all program staff to ensure a seamless transition of the client’s onboarding and orientation
- Supports JESPY House policies, vision, and core values while empowering all clients to direct their own life choices
- Maintain accurate data and general status reports regarding all Intake activity
- Maintain all administrative duties associated with maintaining the day-to-day operations of the Intake service

Other Responsibilities
- Adopts all Agency policies and procedures and attends mandatory in-service programs.
- Reports any incident or accident involving oneself, or Agency clients. Reports all unusual, questionable, or dangerous situations to management immediately. Provides all information, in writing, necessary to assist in completing the required report.
- Immediately reports any suspicion of verbal, physical, sexual abuse, or neglect on the part of clients or any staff to the immediate supervisor and Executive Director.
- Reports immediately any unusual, inappropriate, or potentially risky behavior on the part of anyone visiting an individual at any of the JESPY sites, to the site supervisor, the Director of Operations, or the Executive Director
- Maintains strict confidentiality of organizational and individual information, in accordance with HIPAA standards.
- Demonstrates concern and courtesy to clients, staff, family members and visitors.

Educational Requirements
- Bachelor’s degree in human services or related field preferred or 5 years’ equivalent related experience
Work Experience Requirement
- Experience in residential and/or day habilitation
- Case management experience
- Experience working with DDD and other NJ State social advocacy agencies
- Knowledge of entitlements, i.e., Social Security, DDD, Medicare, etc., and process of obtaining entitlements
- Minimum of 3 years’ experience in Behavioral Health working with Mental Health and/or DDD population strongly preferred.
- Proficient in EHR (Electronic Health Records) systems
- Proficient in Microsoft Office Suite, including Excel and Outlook.

- Knowledgeable of the person-centered approach to service delivery. As an empathic listener, remain patient, listen carefully, and offer appropriate feedback to clients served
- Maintain strict and professional boundaries particularly regarding confidentiality.
- Understanding of learning and developmental disabilities and best approaches to working with clients with disabilities

Position Type/Expected Hours of Work
- Hourly
- Monday through Friday, some weekends as required
- Must have availability of 8am – 8pm, as department schedule dictates
Special Working Conditions/Physical/Requirements – Working Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
- Must be available for coverage on some weekends and after 5pm when required.
- Valid driving license is required
- Must hold a high school diploma
- Valid New Jersey driver's license is required
- Must hold CPR/First Aid Valid Certification
- Successful clearance of NJ background check
- Successful clearance of Drug Screening
- Has fulfilled the necessary training requirements and is in good standing to bill Medicaid for Support and Community Care Waiver (CCW) Services and CDS
