Coordinates infusion and enteral services for patient discharging from inpatient facilities as well as managing therapy changes for existing patients. Communicates regularly via telephone/email/instant messaging with social workers, UICH pharmacy, nursing, inventory, and warehouse staff as well insurance companies. Ability to multitask, attention to detail and flexibility are important qualities for a candidate in this position.
- Coordinate care for patients admitted to the UICH service
- Coordinating delivery method of drug with pharmacist
- Identifying agencies and coordinating provision of infusion and other needed nursing care with the agency
- Providing follow-up communication with nursing agencies, including but not limited to, drug infusion, IV access and care issues
- Performs required documentation to record initial care planning, inventory records and verbal and written communication with referral sources
- Completes chart reviews (as needed) to ensure therapy meets insurance coverage criteria before accepting patient on service
- Enters lab visits in EPIC
- Coordinates changes to existing patients therapy
- Communicates changes with patients, care givers, UICH staff, physicians and nursing agencies
- Modifies delivery tickets as needed according to new orders received
- Enters order and updates patient information on Enteral and Chemotherapy patients requiring an SMN renewal then sends a TO DO message to nurses to complete the process
- Obtain health history and scan into CPR+
- Communicates regularly with insurance companies to verify coverage and benefits as well obtain prior authorizations as needed
Education Requirements
- High School Diploma or equivalent
- Recent employment experience in busy office setting, preferably health care related
- Effective interpersonal and written and verbal communication skills. Excellent telephone skills
- Ability to multitask. Creative problem solving
