POSITION: Residential Coordinator
DEPARTMENT: Residential Transition Services
EXEMPTION STATUS: Full Time Exempt

Position Summary: The Coalition for Barrier Free Living is a non-profit organization that provides direct services, advocacy, and support to individuals with disabilities and chronic conditions to ensure they can thrive while living independently. We promote a philosophy of Independent Living (IL), including consumer control, peer support, self-advocacy, and equitable access of individuals with disabilities and the integration of full inclusion of individuals with disabilities into the mainstream of society. We have a strong focus on equitable access for marginalized communities, including but not limited to seniors, bipolar citizens and LGBTQIA2S+.
General Description: The Residential Coordinator is a voice for self-advocacy, disability rights, and Independent Living by assisting adults with a disability working toward Independent Living communities as well as those needing assistance, finding income restricted housing, Houston Housing Authority Program, transitional housing, and managed care organizations. This position focuses on addressing housing and public service needs of low- and moderate-income communities in and around Houston.
Duties and Responsibilities:
- Understand, subscribe to, and advocate the philosophy and values of Independent Living, which are culturally relevant in education, empowerment, and community integration.
- Network with local, state and national organizations, advocating for and providing community living options.
- Work collaboratively with the consumer to develop a plan of services designed to achieve the consumer’s identified goals.
- Case management duties are performed within specified time frames, including but not limited to completion of assessments of identified for housing relocation assistance, assist in the development of a housing independent plan, maintain accurate and current documentation of evaluation, needs, progress, services, and all other categories of information required in each consumer’s service record (CSR)
- Peer Mentoring
- Ability to work with diverse consumers on a cross disability basis, understand their Independent Living needs, and provide referrals for their community resources.
- Provide orientation on IL philosophy and disability services to new consumers.
- Meet regularly with consumers to discuss progress, challenges, and plan goals.
- Aid in processing documents to determine eligibility.
- Provide services in compliance with HIPPA (confidentiality) requirements.
- Meet all agency and funding source deadlines.
- Keep all appropriate records and submit required reports in a timely and professional manner.
- Perform periodic records review to improve data integrity.
- Track participation, compiling reports and other program data.
Required Qualifications:
- Bachelor’s degree in human services field or four years of related work experience with seniors or persons with a disability.
- Ability to problem solve with creative individualized solutions.
- Ability to travel independently in and around the Houston area.
- Two to four years of demonstrated work experience in service delivery to persons with disabilities in a myriad of ways.
- Ability to perform mathematical calculations using basic skills and/or a calculator.
- Skilled in using MS Office, Excel, and other agency supported software.
- Patience and a positive attitude when under pressure.
- Case management of consumer files.
- Must have reliable transportation.
- Must have a valid state driver license and insurance.
Preferred Qualifications:
- Qualified person with disabilities.
- Experience working in the Independent Living field.
- Experience in developing training curricula and materials and conducting trainings.
- Fluent in a second language such as Spanish, in addition to English fluency.
