Position Summary
The coordinator is responsible for communicating with our nursing team and third-party vendors to schedule couriers, communicate with our third-party vendors for imaging, arrange transportation, communicate with patients, and coordinate all care and needs for our patients in their home while admitted to HCaH. They work closely with nurses, physicians, and team members in other areas of the hospitals to coordinate care. Communication and time management are essential skills for this role. Traveling to patient homes is not required and the coordinator will work solely in the patient care hub. Qualifications:

The Orlando Health Hospital Care at Home program provides acute hospital-level care to adult patients in the comfort of their own homes, allowing nurses the unique opportunity to build relationships and connect one-on-one with their patients and their patients’ families. Our enhanced acute-care model uses portable technology to connect patients to the Orlando Health Patient Care Hub for 24/7 remote monitoring and virtual care. Education/Training
- High School diploma or equivalent.
- Keyboarding skills and knowledge of Microsoft Word and Excel.
- Knowledge of Medical terminology, or Health Unit Coordinator Certification.
