SalaryAre you a recent college graduat looking for a fulfilling career in human services with room for professional development and growth? We are looking for a caring, compassionate, detail oriented, person to train as a supports coordinator. We have two positions to fill in the Chester County Area.
IDD Supports Coordination involves working with families, individuals with differing abilities, and other providers to support individuals in the community. You will meet with families both virtually and in person to develop individual support plans. You also ensure the implementation of the plan and monitor the services provided by other providers.
We are looking for good time management skills, an ability to document activities clearly and concisely, and follow regulations and guidelines. You must have a Bachelors degree with a minimum of 12 credits in the humanities or social sciences, a valid drivers license, and insurance.
Job expectations include, meeting with individuals and providers, documenting all activities to locate, coordinate, or monitor services, working with Teams to provide support, meeting billing expectations of 110 units, and building solid relationships with individuals to ensure they are living Everyday lives, in the least restrictive environment, and receiving all authorized services.
