Position Title:      Housing Case Manager (GRH)

                                   

Salary Status:      Non-exempt

 

Classification:      Grade Level - 7

Qualifications

 

Bachelor’s degree in Human Services or related field. Experience working in the Human Services field as a case worker / Case Manager, advocate or resident assistant required.  Must have a good understanding of the delivery of Social Services and community services, the ability to accept supervision and work with a team of co-workers, work well with clients of a diverse cultural and socioeconomic groups; respect of confidentiality; and able to work within ethical and legal parameters of social services. Ability to work very well independently, with excellent skills in teamwork and collaboration. Knowledge of Harm Reduction/Housing First Model and commitment to that model. Must be willing to support the philosophy, goals and mission of The Salvation Army, able to be certified in Adult CPR and First Aid, pass the background checks and a pre-employment drug test, possess a valid MN drivers’ license without restrictions and meet the MVR drivers’ qualifications .  It must be understood by the incumbent that the provisions of Safe From Harm will apply upon commencement of employment, and also understood that the required Safe From Harm training program must be satisfactorily completed within the first ninety (30) days of employment.

Position Summary

 

The Housing Case Manager (GRH) will work in collaboration with the Top 51 Case Managers in providing services to long-term homeless chronic inebriates. The team utilizes a Harm Reduction/Housing First Model to assist long-term homeless individuals in finding and maintaining housing. Team members will respond to each participant’s unique needs and goals by providing on-going support, advocacy and assistance in accessing community services to ensure stability in housing and integration in the community. This position works in an independent manner with little direct supervision of the Lead Housing Case Worker.

 

 

 

Essential Duties and Responsibilities

 

  • Conduct outreach to potential participants within shelters, streets and community programs.
  • Network with other agencies and individuals to help access housing.
  • Cultivate relationships with landlords willing to provide housing for participants.
  • Maintain a caseload of a minimum of 15 case management participants.
  • Assist clients with completion of necessary forms to access housing.
  • Review with each participant issues of renter expectations, renter/landlord conflict resolution, and tenant rights.
  • Assist in administrating rental assistance.
  • Assist participants to secure and maintain housing, apply for benefits and assist with accessing and moving furnishings.
  • Offer regular and quality contact with participants to maintain trust, to assist in crisis prevention and to assist in meeting individual needs.
  • Develop individual service plan with participants and provide supports necessary to achieve goals.
  • Assist in crisis prevention/intervention when needed to ensure housing stability.
  • Communicate and coordinate services with all other team members. Attend and participate in team meetings.
  • Provide flexible services during times that meet needs of participants.
  • Maintain participant files and develop reports as required.
  • Assist in developing and leading daily group programming.
  • Input all client information into Service Point.
  • Inform Housing Program Manager(s) and Lead Housing Case Worker of programmatic issues and concerns regarding client health, safety and well-being concerns.
  • Provide required statistics (TSA stats, client’s files, United Way, etc.) to COD.
  • Participate in office and department meetings, committees and other meetings as appropriate; participate in community meetings and networking.
  • Attend educational opportunities when appropriate.
  • Maintain positive work atmosphere by behaving and communicating in a manner that fosters good relationships with co-workers and supervisors. This would include, but not limited to such actions: conflict resolution in a professional manner; courteous treatment of staff, visitors and clients; respect of others property and person and appropriate communication to and about co-workers and supervisors.
  • Perform all other duties as assigned, to include the annual Kettle Campaign.

 

 

Working Conditions

 

There are normally no hazardous or significantly unpleasant physical working conditions, with the exception of the occasional emotional distress or turmoil (due to varying situations) with and between staff and/or clients.  The essential duties and responsibilities of this position cannot be performed from home.

Risk Level 2 - The tasks or activities conducted by Risk Level 2 workers do not involve reasonably anticipated exposure to blood or other potentially infectious materials, but could result in such exposure in non-routine, emergency situations.

Hearing

  • Able to hear average or normal conversations and receive ordinary information.

      Repetitive Motion   

  • Movements frequently and regularly required using the majority of the body.

Physical Strength   

  • Variety of Work; sitting approximately 50% of the time, with some prolonged standing, walking, bending, climbing
  • Exerts up to 30 pounds of force occasionally.                                              

Talking

  • Especially where one must frequently convey detailed or important instructions, information, and ideas accurately, loudly, or quickly.

Travel

  • The incumbent will be required to travel by personal vehicle, organization vehicle or other mode of travel as required supporting the mission of The Salvation Army.

Visual Abilities   

  • Average, ordinary, visual activity necessary to prepare or inspect documents.

 

Mental Activities and Requirements of Position

 

Mathematics Ability

  • Ability to perform basic math skills, addition, subtraction, multiplication, division, fractions and percentages.
  • Ability to prepare budgets

Language Ability

  • Ability to fluently speak read and write in English.
  • Ability to read a variety of books, magazines, instruction manuals, atlases, and encyclopedias.
  • Ability to prepare memos, reports, and essays using proper punctuation, spelling and grammar.
  • Ability to communicate distinctly with appropriate pauses and emphasis; correct punctuation and variation in word order; using present, perfect, and future tenses.

Reasoning Ability

  • Ability to deal with a variety of variables under only limited standardization.
  • Able to interpret various instructions.
