Job Summary
Program Overview: Credent Care uses case management and housing support to move
chronically homeless individuals and families into permanent housing and provides long-term
rental assistance and case management support to help people stabilize in housing and not
return to homelessness.
Position Overview: The Housing Case Manager maintains a caseload of 10 to 20 households in
permanent supportive housing and works with clients to ensure long-term housing
sustainability. Case management is specifically focused on housing maintenance and stability:
paying rent, interacting positively with landlords and neighbors, and managing behaviors that may
accompany mental illness or substance use so that they don’t interfere with success in housing,
developing crisis plans, connecting with appropriate community resources, and supporting
clients’ individual housing goals.
Responsibilities:
Work with each client to develop and progress on goal plans according to individual
goals and objectives
Maintain contact with all housing clients on a scheduled basis, appropriate to their
particular status and needs
Ensure that client interactions are person-centered, and goal-focused and reflect each
client’s goal plan.
Provide transportation to clients when required.
Make referrals to supportive services in our community as appropriate and support the
client in navigating this process.
Effectively use HUD tools and ensure compliance with data standards and record-
keeping as required by HUD and Company
Document all client interactions and maintain accurate paperwork/reports
Perform annual re-certifications and inspections for each client and accurately update
files as necessary.
Attend regular case management meetings and other specified meetings/trainings as
required.
Communicate effectively in oral and written forms with clients, their families, their
significant others, coworkers, supervisors, other service agencies, and the community
Operate proficiently a personal computer, fax machine, printer, and copier
Qualifications
Bachelor of Social Work or related degree; graduate degree with clinical focus preferred
Prior experience with homeless populations or populations with high clinical needs
Reliable vehicle/valid drivers license and ability to transport clients.
Knowledge of benefit/assistance programs in Minnesota
Experience working with vulnerable adults.
Housing or Human Services experience
Ability to quickly learn new software systems.
