Job Summary
The role of the Host Home Care Coordinator involves recruiting, selecting, and orienting host families to be a part of the program—along with providing on-going support for families enrolled in the program. The Host Home Care Coordinator will also provide intensive individualized case management services for teens participating in the program.
Job Duties/Responsibilities

- Assist in the development of procedures and practices for the Host Home Program in alignment with state and national best practices. Coordinate with Youth Services Administrator and CCWW Director to conduct community outreach, marketing, and coalition building.
- Implement core principles and evidence-based practices that include Positive Youth Development, Trauma Informed Care, and Youth Mental Health First Aid.
- Recruitment and engagement of youth ages 16 – 17 with experience of homelessness to participate in the Host Home Program. Conduct community outreach and build high-quality relationships with youth to understand the personal experiences of children and youth impacted by homelessness and trauma.
- Recruitment, selection, and orientation of Host Home families.
- Interview and conduct home-site visits with potential Host Home families.
- Complete Host Home Program intake with youth and Host Home family.
- Create Individualized Care Plan for each youth that focuses on family reconciliation, obtaining safe and stable housing, promotion of social and emotional wellbeing, education and employment, permanent connections & aftercare supports.
- Provide information and referrals as needed and link youth with community resources ensuring ongoing high quality of care.
- Provide support to youth and Host Home families if problems arise in the home.
- Assist with transporting youth to and from scheduled appointments and other designated activities as needed.
- Provide representation on CCEW Host Home work at Anchor Community Initiative (ACI) meetings.
- Provide follow-up and aftercare services as needed for youth discharged from the Host Home Program, with guidance from the Youth Services Administrator.
- Maintain client files per agency requirements, including organizing information to facilitate audit trails and procedures. Perform client file destruction in accordance with agency policies and procedures.
- Provide clear, concise, and empathetic information in all written and verbal communication interactions.
- Provide accurate, complete, and timely data, and reports to Youth Services Administrator and CCWW Director as needed.
- Other special projects as assigned by the Youth Services Administrator or CCWW Director.
- Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine.
- Perform related functions necessary to support the mission and core values of Catholic Charities.
- Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
- Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.
- Excellent written and verbal communication skills, and ability to present to diverse audiences, specifically racially, ethnically, and socioeconomically diverse communities.
- As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.
Job Qualifications
To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job. Bi-lingual preferred (Spanish & English) and experience working with the Latin community preferred.
Education/Experience: to perform this job successfully, an individual must have:
- A BA in Social Work or related field preferred, or 3 years of lived experience equivalency.
- Familiarity with local issues related to youth and young adult homelessness, and/or knowledge of youth-serving systems and homelessness in general.
- Ability to work with diverse communities and commitment to supporting inclusion, both internally and across our efforts to serve all youth and young adults experiencing homelessness.
- Knowledge of social service and educational resources in the greater Walla Walla area.
- Proficiency with Microsoft Office applications and ability to navigate database software platforms with ease.
- Ability to organize, set priorities, monitor progress towards goals, and track details.
- Excellent written communication skills – proofreading, punctuation, composition, and grammar.
- Excellent interpersonal skills and the ability to work with confidential and sensitive information while assisting staff, volunteers, and the public.
- Ability to work independently using high-level discretion, maintaining the highest degree of confidentiality, and ability to manage multiple details and deadlines. Must be a self-starter and can exercise initiative.
Certificates/Licenses: To perform this job successfully, an individual must have a valid Driver’s License and the ability to drive for work use. Successfully pass background check applicable to position.
