Radiant Hospice seeks a compassionate and motivated Business Development Liaison/Patient Care Navigator responsible for managing all aspects of Hospice organization's marketing and community education and awareness, including establishing and maintaining positive relationships with customers, community partners, and referral sources, responding to customer requests and concerns. The Business Development Liaison/Patient Care Navigator works to improve patient outcomes by providing patients and families with resources for value based care with the goal of preventing unnecessary ER visits, reducing unnecessary hospital readmissions, and improving access to Hospice Care when timing is appropriate for the patient and their family.
QUALIFICATIONS:
- Educational/Degree: Bachelor's degree in Marketing, Business Administration, or related field OR have a baccalaureate degree in psychology, sociology or other field related to social work (MSW), and at least one year of social work experience in a health care setting and be supervised by a MSW from a school of social work accredited by the CSWE and who has one year of experience in a health care setting.
- Knowledge/Skills/Ability: Ability to educate about hospice program benefits, market the hospice program consistently, and deal tactfully with customers and the community. Demonstrates excellent customer service skills, communications skills, negotiation skills, and public relations skills. Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities
- Experience: At least two years of experience in health care marketing, preferably in hospice care
- Valid driver’s license, proof of acceptable automobile insurance coverage, and reliable personal transportation
- Excellent demonstrated critical thinking skills and superior clinical judgment
- Excellent computer skills
- Reliable cell phone with local number and coverage
- Ability to work independently and manage time effectively. Ability to make accurate, and at times, quick judgments.
Job Duties (including but not limited to):
- Identify and assist potential hospice patients and their families with navigating the healthcare system with the ultimate goal of improving patient outcomes by providing patients and families with resources for value based care, preventing unnecessary ER visits, reducing unnecessary hospital readmissions, and improving access to Hospice Care when timing is appropriate for the patient and their family
- Establishes and maintains positive working relationships with current and potential referral sources. Completes all daily tasks of marketing and implementation of marketing initiatives.
- Builds and monitors community, customer, payer and patient perceptions of Hospice as a high-quality provider of services. Continuously conducts market assessments and develops a comprehensive marketing plan designed to meet agency goals.
- Employs marketing and promotional initiatives to achieve agency projections.
- Follow strategic planning to provide opportunities for additional or improved services to address customer needs.
- Maintains comprehensive working knowledge of Hospice markets including government agencies, major payer groups, key referral sources, and competitor’s market positioning.
- Maintains comprehensive working knowledge in the field of marketing and shares information with appropriate organization personnel.
- Maintains comprehensive working knowledge of community resources and assists customers in accessing community resources should services not be provided by Hospice
