Work Schedule: 8 am - 4 pm, M - F, one-hour unpaid lunch. Adjustments to schedule with supervisor approval. Early mornings and evenings as needed.
POSITION SUMMARY
Assists families in meeting their individual needs as identified in the Family Plan and works with families to provide increased parent knowledge of child development, improve parenting practices, and/or home intervention services. Will provide home visits and coordinate services to the family and become a major support for the parent(s).

• Administer childhood screenings and family assessments; facilitate and participate in the development, review, and evaluation of Family Plans to address the needs of the family; and implement parenting curriculum and parent/child activities.

• Schedule and visit client homes to provide information on program services, requirements, and procedures.

• Develop and maintain up-to-date individual case files for all referrals and eligible individuals in a confidential manner in the Ohio Comprehensive Home Visiting Integrated Data System (OCHIDS), or other databases as directed or required.

• Participate in professional and workforce development requirements and opportunities.
Policy Development/Program Planning Skills:
• Use quality improvement (QI) and performance management (PM) processes and/or techniques to improve the effectiveness of home visiting program including but not limited to creating, implementing, and evaluating performance standards and identifying, implementing, and assessing QI processes.
• Develop lesson plans using parenting curriculum.
Public Health Science Skills:
• Compile and maintain required program statistics. Assure that each child is registered in the appropriate state tracking system.
• Initiate and facilitate required transition activities.
Other:
• Responsibilities include other duties as assigned including but not limited to Accreditation requirements and responding to emergency events.

SKILLS & ABILITIES
Education:
High School Diploma or equivalent: As defined under Ohio Administrative Code 3701-8-04 (D)(1)
Experience:
None
Computer Skills:
Microsoft 365 required. SharePoint and Ohio Comprehensive Home Visiting Integrated Data System (OCHIDS) obtained upon hire.
Certifications & Licenses:
Valid driver's license required.
Must be credentialed as a Home Visitor and must complete the required home visiting training within six (6) calendar months of hire. Must maintain credentials without lapsing and renew credentials every two (2) years.
National Incident Management System (NIMS)/Incident Command System (ICS) 100, 200 and 700.a certification to be obtained within six (6) months of hire.
