OhioGuidestone provides a complete continuum of prevention services and mental health and substance use treatment, focusing on responsive person-centered care to approximately 26,000 people each year through telehealth and locations across the state.
The Home Visitor is responsible for providing home visits to clients enrolled in the Help Me Grow program to assist families with the goal to provide support, encouragement, information and resources to promote positive parent-child interaction.
Duties/Responsibilities:
- Provides regular home visits to enrolled Help Me Grow families with children from prenatal to three years to provide: direct service to families as required; referrals to service providers; supervision, family support, crisis intervention, mediation, information and education; education that focuses on parent/child interactions, developmental instruction and proper education setting as needed.
- Plans effectively for each visit and teaches effective parenting and child development skills.
- Assists families with developing and attaining goals developed on their Family Plan and making formal and informal connections to appropriate community services throughout the program.
- Provides parents with information on disciplining, behavior management, household management and communication and relationship enhancement when needed.
- Performs related administrative responsibilities; maintains client information, reports, records, and other documents; responds timely to emails and phone calls; attends various trainings and meetings; communicates with other agencies, departments, and vendors regarding required paperwork, contract compliance and consults with other county agencies and/or supportive services as necessary.
- Supports the completion of funder requirements and program assessments.
- Organizes and maintains written documentation for participants, employer, and educational institutions in case notes to ensure they are complete, current, accurate and disseminate according to program and department requirements.
- Maintains ongoing contacts (phone, meetings, and conferences) with other professionals involved, including collateral contacts with community resources/services
- Completes and updates various assessment tools and data entry into the required data system, accurately and within required timeframes.
- Utilizes curriculum and implements model as required by program.
- Maintains the confidentiality of client or collateral interaction within Agency defined standards.
- Participates in staff training, supervisory and staff meetings.
- Maintains regular and reliable attendance.
- Exhibits positivity, flexibility and a willingness to provide in service training and take on new responsibilities as requested or required.
Required Skills/Abilities:
- Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
- Computer literacy skills, including Windows applications, Microsoft Office Suite, and basic database usage, as well as basic typing.
- Must be able to exchange information with participants and/or families in various settings regarding program plans and performance of participant.
- Organizational skills and attention to detail.
Performance/Physical Requirements:
- Work consist of walking, bending, stooping, ascending/descending stairs, and remaining in a stationary position 50% of the time, including positioning self to an equal level in order to assist children of all ages.
- Work environment is based in the community and can include an agency office, school-based setting, child care facilities or participant's homes.
- Must be able to drive frequently due to work varying work environments including agency office, school based, community based locations which may include participant homes in various communities.
- Works flexible hours, which can include days, evenings, and weekends, as desired to meet the needs of clients and the OhioGuidestone and is available for crisis management by phone as needed.
- Interacts verbally with coworkers, representatives from outside agencies, and families.
Qualifications:
- Must have a minimum educational level of a Associates degree* in Early Childhood, Human Services, or a related field, Bachelor's Degree preferred.
- Must have a valid Ohio Driver's License with a safe driving record and valid insurance.
- Ability to take and pass a physical exam, tuberculosis test and drug screening.
- Employment is contingent upon clear results of a thorough background check.
- *Funding sources may require OhioGuidestone to hire an advanced degree
