Catholic Charities is seeking mission-driven candidates to join our Help Me Grow Home Visiting Program as a Home Visitor. In this role you will work with pregnant mothers, caregivers and their children to promote healthy child development and positive interactions between parents/caregiver and child. This position is responsible for engaging families who have been referred to the program; completing home visits, develop goals, conduct screenings/assessments, facilitate parenting education activities and making referrals to community resources.

Qualified candidates must have at least an associate degree in urban studies, human services, Early Childhood, social work, psychology or 2+ years experience in human services, Early Childhood or similar field. A valid driver's license with a good driving record also required.
Program Statement:
Lake County Help Me Grow Home Visiting Program is a voluntary program that gives pregnant women and families the necessary resources and skills to raise children who are physically, socially, and emotionally healthy to learn. The goals of the program are to 1. Improve maternal and child health. 2. Prevent child abuse and neglect. 3. Encourage positive parenting; and 4. Promote child development and school readiness.
What You Will Do:
- Build relationships with families to assess their strengths, utilize case management skills determine their needs, and work together to develop goals and connect the family to essential resources.
- Work as an integral member of the Help Me Grow Home Visiting Program by participating in ongoing recruitment as well as completing enrollment applications accurately and timely to determine the eligibility of families for the program.
- Analyze and interpret data to effectively monitor and identify critical areas that will allow the whole family to thrive including family services, health & nutrition, physical and mental health.
