Help Me Grow Home Visitor

MINIMUM QUALIFICATIONS:

Completion of an associate’s degree in a field of study related to early childhood, education, human services, social services, or related fields; a minimum of six (6) months related experience; or any equivalent combination of education, experience, and training which provides the required knowledge, skills, and abilities. Must be able to read, write, speak, and understand the English language.

LICENSURE OR CERTIFICATION REQUIREMENTS: Must possess and maintain a valid Ohio driver’s license and have reliable transportation; must meet the requirements for being insured by the Health District’s insurance provider. Must obtain Home Visitor credential through the State of Ohio within one (1) year of hire.

JOB RESPONSIBILITES:

• Drives motor vehicle to conduct regular, on-going visits with families of children ages 0-3, and with pregnant women

• Conducts visits using program-approved curriculum

• Completes developmental and other screenings for children and parents or caregivers

• Facilitates development of family goal plans

• Provides parenting education and support to improve child well-being, to include topics such as discipline, behavior management, safety, and child development

• Facilitates delivery of health/medical, educational, and social services by making referrals to appropriate service providers and community resources; follows up on referrals made.

• Maintains regular contact with families to provide support and guidance

• Completes and maintains accurate documentation of all family contacts and visits (electronic data systems and/or hard copy), track and report data

• Maintains family/client confidentiality

• Conducts outreach and program promotion by attending community events and providing program information and/or literature to the community

Job Type: Part-time

Pay: From $18.00 per hour

Expected hours: 24 per week
