Job Summary: The Patient Care Coordinator/Intake will oversee the coordination of Home Health Patient Care, including scheduling of clinical staff and obtaining insurance authorization, in accordance with policies and procedures. The Patient Care Coordinator works closely with Talent Acquisition and the clinical managers to understand and secure the needs of the Home Health Patients. Utilizes patient portals for admission; Prepares work schedules, makes job assignments, and obtains insurance authorization. · In combination with Clinical Managers, manages the Home Health clinical field staff in compliance with policies and procedures, regulations or applicable regulatory agencies, and standards of best practices. · Coordinates care across internal and external departments and agencies. · Represents the organization to clients and referral sources. Duties/Responsibilities: · Assigns staff to appropriate patients · Cultivates, maintains, and enhances relationships with patients, staff, and referral sources. · Identifies, collects, and reports relevant statistical data to reflect the activities of the clinical staff. · Assists staff in various functions required to meet the needs of the patients. · Manages clinical field staff in combination with Clinical Managers Required Skills/Abilities: · Effective management skills, verbal and written communication skills, interpersonal skills, leadership skills, analytical skills, organizational skills, math and budgeting skills, accurate typing and data entry skills. · Respects the confidentiality of information learned about patients. · Performs job duties in compliance with the policies and procedures of Home Health care, Medicare, and the company. · Courteous and respectful toward fellow employees, patients, and referral sources. · Promotes positive public relations for the office. · Motivates staff and maintains a high level of morale.
