Agape’s Way is seeking a Home Care Coordinator/Scheduler to join our award-winning home healthcare team! As a Home Care Coordinator/Scheduler, you'll organize caregiver schedules, match them with clients, ensure smooth communication between all parties, also schedule patient appointments and dispatching transportation to ensure patients are transported to and from scheduled medical appointments such as cardiology, nephrology, neurology, etc
Your role is crucial in maintaining high-quality Home care services and client satisfaction.
Responsibilities:
· Will work M – F from 8 am – 5 pm
Good communication skills with participants, caregivers, family, guardians, DPOAs, team and external offices to ensure accurate appointments are well coordinated and met
Verbal communication will include in person, telephone, EMR chat, Outlook, Teams, or other means of communication
· Implement HR policies and procedures for time and attendance, minimizing overtime, and ensuring compliance.
· Manage caregiver recruitment, hiring, and records.
· Coordinate client services and maintain confidentiality.
· Participate in performance improvement and HR strategy.
· Prepare and submit payroll, report HR metrics.
· Demonstrate effective communication, participate in on-call rotation.
Qualifications:
• Minimum 21 years old, valid driver's license.
• College degree preferred.
• 1 year supervisory/admin experience in healthcare or business.
• Knowledge of state regulations, background check eligibility.
• CPR certification, HHA training completion.
• Strong communication, organizational, and leadership skills.
• Ability to work independently, prioritize tasks, maintain confidentiality.
• Proficient in Microsoft Excel and Word.
• Flexibility for evening/weekend work.
